This form records any sport-related injury or incident that occurs during handball training, matches, warm-ups, or cool-downs. Every submission helps protect athlete wellbeing, supports injury prevention, and contributes to injury surveillance across Irish and Scottish handball.
Please complete the form as soon as possible after an incident. It only takes a few minutes, and every report helps build a safer sport.
This project covers senior players aged 18 and over only. Please do not submit reports for any player under 18.

You’ll be asked to:
Identify yourself and the injured athlete
Record the date, venue, match/training context, and specific action when the injury occurred
Indicate if the athlete continued playing or not
Select the injured body region and side (L, R, or B)
Describe the injury mechanism (e.g. contact, overuse)
Note any suspected or confirmed concussion
Add relevant exposure data (minutes played, training frequency, etc.)
Four weeks after initial submission, you will receive a follow-up link via email to complete report.
The form is completed and submitted online. There is no need to download, print, or email anything. After you submit the initial record, a short follow-up is sent automatically about four weeks later to capture the athlete's recovery.
All data is handled in line with GDPR, Sport Ireland's Privacy Policy, and Sport Scotland's General Privacy Notice. Data is anonymised for research and monitoring, supporting our shared commitment to player welfare and injury prevention. By submitting the form, you confirm the athlete agrees to their anonymised data being used for these purposes.
